Magazine seeks to honor independent agents

Winston Churchill said, “We make a living by what we get; we make a life by what we give.” Through its 6th annual Community Service Award, The Rough Notes Company seeks to identify and honor independent agents who have enriched their communities through philanthropic deeds.
To nominate someone, simply complete the Nomination Form on the following page and, with the required documentation, mail to The Rough Notes Company on or before December 31, 2004. Your input allows us not only to recognize deserving agents, but also to honor their charitable organizations.
The winning charity will receive a $5,000 donation from The Rough Notes Company, and honorable mention charities each will receive $1,500. Each winning agent or agency will receive a beautifully engraved eagle statue that will be presented during an awards dinner and ceremony in Indianapolis in 2005, in conjunction with Rough Notes magazine’s 16th annual Marketing Agency of the Year award ceremony.
Here are just a few of the Community Service Award agents/agencies that have been recognized through the years:
• James Colangelo and Richard Morello started Family Reach Foundation after losing children to pediatric cancer. This organization financially supports families struggling with the high costs of caring for children with pediatric cancer.
• Stephen Heinen was the driving force behind building a new facility for Challenged Child and Friends, Inc., which provides care for children afflicted with numerous types of physical and mental disorders.
• Steve Brown and his entire agency began a program known as HBC Feeds, which provides food packages for the needy in conjunction with Los Angeles Family Housing.
• John Pollock, along with employees of his agency, financially supports more than 50 charities including the American Cancer Society’s Relay for Life.
• C. Gary Stewart has worked for more than 20 years in support of the American Cancer Society by giving classes on cancer screening and seeking a cure for this disease.
• Ben Bates and Dale Hewett have financially supported and been on the front lines of the Rodeheaver Ranch, which provides living facilities for boys who are orphaned or have been in dysfunctional living situations.
• Tad Krug has made major service contributions to Youth Home, Inc., a nonprofit agency serving severely emotionally disturbed children and their families.
• Jack Allen demonstrates his passion for public education and a firm commitment to the children in his community by being an informed voice for public schooling at business lunches, boardrooms and civic gatherings.
• Edward Joseph was the first person to contribute to and chair a hospice home in his local community; he was active in this project from the feasibility study until the first location became a reality.
As you can see, independent agents and agencies perform community service that covers the gamut of charitable organizations. There are some common threads among our winners: None seem to want public recognition for their extraordinary and wonderful deeds! All seem to feel a responsibility to help those in need in the cities and towns where they make their living, all in hopes of strengthening their communities through giving. n

Award Conditions and Prerequisites
Purpose: To recognize and award an independent property/casualty broker, agent or agency that has demonstrated extraordinary community service within his/her/their local, regional or national community. And, to present the winning community service project with a check in the amount of $5,000 to help further its special cause(s).
Criteria: Nominees will be judged on their overall contribution (funds raised, people helped, volunteerism, etc.) to their community service project and/or initiative, and the positive impact (both short-term and long-term) that their efforts have made toward their community service project.
Nomination Period: Nominations will be accepted at the offices of Rough Notes magazine by U.S. mail, overnight delivery or hand delivery up to and including December 31, 2004. No nomination forms will be accepted after December 31, 2004. Please note that no facsimiles will be accepted.
Conditions:
1. Nominators must complete the nomination form in its entirety and provide all supporting documentation;
2. All nomination forms and supporting documentation must be received at the offices of Rough Notes magazine no later than December 31, 2004;
3. No facsimiles will be accepted;
4. All nomination forms and supporting documentation become the exclusive property of Rough Notes magazine. Further, the nomination form and supporting documentation may be used at the sole discretion of Rough Notes magazine to publicize the award and award recipient, and to promote the Community Service Awards program;
5. A letter from the community service project verifying and describing the project must accompany the nomination form;
6. Incomplete nomination forms or nomination forms without supporting documents will not be considered;
7. Nominators may nominate themselves or their agency.
Selection: Designated individuals from The Rough Notes Company
and selected others will review all nomination forms and supporting
documentation and select a winner and honorable mention recipients. The winner and honorable mention recipients will be honored and presented with their awards at the Rough Notes Magazine Awards Ceremony in Indianapolis, Indiana, in conjunction with the 16th Annual Rough Notes Magazine “Marketing Agency of the Year” award dinner. A representative of the winning community service project will also be invited to attend this function to be honored and presented with a check.
Awards:
1. The winning community service project will be presented with a check in the amount of $5,000 in the name of the award recipient.
2. The winning broker, agent or agency will be presented with a
sculptured engraved eagle and a reproduction of the check to the
community service project.
3. Honorable mention recipients will be presented with a sculptured engraved eagle and a check for $1,500 for their community service project.
4. Rough Notes magazine will publish an article to publicize the community service project, the award winner and the honorable mention recipients.
