The Rough Notes Company

Community Service Award

Nomination deadline is approaching

The independent insurance industry is comprised of generous individuals who perform philanthropic deeds and services within their local communities. The Rough Notes Company, through its annual Community Service Award, seeks to recognize agents or agencies that volunteer their time, efforts and financial resources to help those who are less fortunate in hopes that their contribution will positively affect others’ lives.

With the December 31 deadline looming, time is running out to nominate an agent or agency for the 2006 Community Service Award. Please help us identify deserving insurance agents or agencies that have touched the lives of those in need and have made the community a better place in which to live. You can nominate yourself or your agency as well. Use the following nomination form (along with supporting documentation) to recommend an insurance colleague or agency that exhibits the philanthropic spirit we seek.

The winning charity or community service project will receive $5,000 in the name of the winning agent or agency. The honorable mention recipients’ charities or community service projects will receive $1,500 in the winning agent or agency’s name. In addition, the winners will receive an eagle statue with an engraved plaque recognizing their contributions to the less fortunate.

The winner and honorable mention recipients will be honored at an awards dinner in Indianapolis in early 2006 in conjunction with Rough Notes magazine’s 16th annual Marketing Agency of the Year celebration. The April 2006 issue of Rough Notes will feature a special segment on all of the winning agents and agencies and their special community service projects.

Thank you for helping us honor those worthy independent insurance agents or agencies for our 2006 Community Service Award. *
